| Details: | SUMMARY: Sanitary survey corrective action/corrective action plan DETAILS: ODA received written documentation from the water system on 10-8-2015 that one of the significant deficiencies noted during a sanitary survey conducted on 5-12-2015 has been corrected . Specifically, a Coliform Sampling Plan was completed on 7-15-2015. Mr. Yuns approved corrective action plan for the two remaining significant deficiencies, no Operations and Maintenance Manual and no Emergency Response Plan , is that he will have them completed by 10-15-2015 and submit written verification to the regulatory authority by 5:00 p.m. . on 10-15-2015 that the documents have been completed and the dates they were completed. ACTION NEEDED: Submit written verification to the regulatory authority that the system has completed an Operations and Maintenance Manual and Emergency Response Plan and the datesthey were completed by 5 :00p.m. on 10-15-2015. | ||
