| Details: | SUMMARY: Deficiency follow up for water survey conducted on April 16th 2015. DETAILS: The significant deficiencies and rule violations noted are as follows:1. OAR 333-160-0050(6)(a)(M) The drain end on the reservoir requires screening or a flap valve. Provide it, or confirm that it already exists.Corrections are as follows:1. The drain end on the reservoir has been screened.2. A coliform sampling plan has been submitted. 2. OAR 333-160-0036(6)(b)(G) A coliform sampling plan is required that lays out when and where routine and follow-up sampling will occur. . ACTION NEEDED: Operator should note that a copy needs to be present on site for coliform sampling plan, O& M manual, and emergency response plan. Survey deficiencies have been completed. | ||
