| Details: | SUMMARY: I called Kevin to discuss the raising of the Plan Review Fee from$150 to $825 on January 1, 2016, and was able to leave a message. I also emailed Kevin to urge him to send in a fee before December 31, 2015, also an annual nitrate test is also required. I have not had any response regarding either item .
