| Details: | SUMMARY: : Regulatory Assistance DETAILS: A sanitary survey was completed on August 5, 2015 as part of reactivating the store as a PWS . The system had multiple significant deficiencies . The following results have been submitted to correct monitoring deficiencies 1) coliform results , 2) nitrateresults , 3) arsenic results. The following deficiencies have not been corrected 1) installwell vent, 2) protect well from flooding, 3) coliform sampling plan , 4) emergencyresponse plan , 5) Operations and Maintenance Manual ACTION NEEDED: Correct remaining significant deficiencies. | ||
