DETAILS: 1-8-16 ---Steve called to request an extension to the Corrective Action Plan addressing the uncorrected deficiency from the 5-11-15 water system survey. I asked that he resubmit a corrective action plan in writing and send it to our office. I agreed to his request for an extension to 2-15-16, providing he submits the CAP in writing.
