| Details: | SUMMARY: Regulatory Assistance DETAILS: System failed to report results for 5 routine samples in March, 2015, and failed torepmi 3rd quarter routine coliform results. A notice was mailed to the system and the notice was returned as undeliverable. A site visit was made on 1/08/16 and found the store closed, vacant, and the interior gutted. A Water System Information form was submitted to deactivate the system. ACTION NEEDED: No further action required. | ||
