| Details: | SUMMARY: 4-LOG Treatment Confirmed DETAILS: System had E-coli detected in source sampling completed on 12/5/12 for Well #1 (SRC-AA - L38446) and Well #3 (SRC-AC - YAMH754). System was subsequently placed on a boil water notice due to confirmed E-coli in Well #1 only based on confirmation samples taken on 12/6/12. Both active sources (well 1 and 3 (well 2 was off-line)) were considered adequately constructed per OHA geologist Tom Pattee. System had been previously notifed that Well 2 was inadequately constructed and could pose a risk to the other wells if not properly abandoned - see contact report dated 12/17/12. Final Approval was granted for SRC-AA (Well #1 - L38446) and SRC-AC (Well #3 - YAMH754) on 11/20/13 under PR#195-2009 (SRC-AB (Well #2 - L45735) was inactive and not included in the approval). A 4-log viral disinfection treatment system was approved with a minimum chlorine residual at EP-A of 0.2 mg/l in the same Final Approval LTR dated 11/20/13 under PR#190-2012. System started GWR Compliance Monitoring 1/1/13. ACTION NEEDED: System to continue daily entry point monitoring at EP-A to demonstrate that 0.2 mg/l chlorine residual is being met for the 4-log viral inactivation treatment credit. System recommended to pursue formerly abandoning or reconstructing Well #2 due to inadequate construction and risk it poses to Wells #1 and #3. | ||
