| Details: | SUMMARY: TC+ routine sample taken 11/18/15 DETAILS: I received an alert for a total coliform positive routine distribution system sample taken on 11/18/15 at the 204 3rd stand pipe sample location. I called and left a voice mail for Jim on his cell number about the results and sent a follow-up e-mail on 11/20/15 stating that 3 distribution system repeat samples need to be taken within 24 hours. A triggered source sample also needs to be taken from each of wells 1-5 from the City of Dayton. 5 temporary routine samples will also need to be taken from the City of Lafayette in the month of December. Provided all temporary and triggered samples are absent of bacteria, normal sampling can resume beginning in January. In the e-mail, I also provided the after-hours contact info for our public health duty officer (PH Duty officer cell: 971-246-1789, Pager: 503-938-670) ACTION NEEDED: A triggered source sample needs to be taken from each of wells 1-5 from the City of Dayton. 5 temporary routine samples also need to be taken from the City of Lafayette in the month of December.
