| Details: | SUMMARY: Nitrate EP-B Violation DETAILS: While reviewing violations in SDWIS, I noticed that GTF received a 5 point violation for not sampling nitrate at EP-B. I conducted a survey in 2014 and found Well #2 to be installed without plan review. Scott Curry reviewed the system and assumed two entry points. My determination is that there is one entry point. I am preparing an Entry Structure Diagram to submit and make the system one entry point. ACTION NEEDED: Submit appropriate documentation to compliance to show one entry point. | ||
