| Details: | SUMMARY: Major PR Not Approved DETAILS: SDWIS reads under site visits, that Jasper Knolls has not gone through major modification for plan review. I did a survey in December 2015 and stated that Plan Review #42-2013 was completed and approved by Carrie Gentry. Her letter is dated 12-2-15. Ron called and asked about the deficiency and I told him the plan review was approved.Please note this significant deficiency as corrected.Thank you. ACTION NEEDED: None | ||
