Details:	SUMMARY: Reviewed Well Deficiencies with Operator DETAILS: Delivered compliance letter on March 22, 2016 to operator and discussed options for remediation of the casing seal construction issues. Reiterated to the operator that an action plan needs to be sent to our office within 30 Days. ACTION NEEDED: Operator will be working with his options and figuring out his course of action. He must get an action plan to Yamhill County Public Health within 30 days of March 22, 2016 (by April 21, 2016).
