| Details: | SUMMARY: Survey Deficiency Final Follow Up DETAILS: During the 8/20/15 survey, the access to the well was locked and Melissa Kaufman was unable to verfify raw sample tap, screened vent and integrity of the well seal. On 12/8/15, operator Norman submitted pictures of the well head showing the raw sample tap. Melissa made another site visit and verified a screened vent, raw sample tap and that the well seal was intact. ACTION NEEDED: These deficiencies have been corrected. | ||
