| Details: | SUMMARY: Survey Deficiency Follow-up DETAILS: Mr. Haverskate stated that his work duties changed and he was on a sabbatical. I emailed him the survey letter and survey from October 2014. We agreed to extend the deadline to June 30, 2016 to have the remaining survey deficiencies addressed. ACTION NEEDED: Finish operations and maintainance manual, create a coliform sampling plan, complete the Emergency Response Plan, and submit plan review. | ||
