Details:	SUMMARY: Violations DETAILS: Water system has been receiving violation notices for nonreporting of monthly source assessment sample tests. The water system was required (2014) to sample 12 consecutive months for source water assessment and has done only 6 nonconsecutive samples. ACTION NEEDED: The water system was directed via email to begin taking monthly source water assessment samples until being notified otherwise.
