| Details: | SUMMARY: Deficiency Follow Up DETAILS: Still working with system to wrap up deficiencies noted during November survey. Called Butch again this morning and discussed need for DRC form and contract, ASR, CCR and raw sample tap for well. Butch said he received a call from Dottie Reynolds as well reminding. He said he would definitely work on it in the next couple days. ACTION NEEDED: Submit missing documents and install raw sample tap for well. | ||
