| Details: | SUMMARY: Level 1 assessment May 2016 DETAILS: The system had 3 repeat samples and 1 triggered source sample collected on 5/23/16. All were positive for total coliform bacteria. The system was sent a Level 1 form to fill out. This is a documented issue with the source and the system is working on a treatment option. ACTION NEEDED: Return the level 1 form to OHA by June 26th, 2016. 3 routine samples are due in June. | ||
