Details: SUMMARY: Survey deficiencies corrected DETAILS: All survey deficiencies have been corrected. Written protocols for under-certified operators was corrected on 3/23/16, and the cross-connection testing records not current was corrected on 6/14/16. The cross connection control device was removed from the system, as it was obsolete. ACTION NEEDED: No futher action needed.
