Details:	SUMMARY: Survey Deficiency Follow-up DETAILS: On 8-11-16 I called and left a message for Mike Haverskate regarding the deadline of 6-30-16 that we agreed on to have the outlined deficiences addressed for I had not heard any update and it was almost two months past the deadline. I did not receive a response so I called Sean M. on 8-23-16 expressing my concern. On 8-24-16 Nate Brown, the new Facilities Manager, left a message expressing that Mike is still working on the Operations and Maintenance Manual and recently had surgery and was recently out of the country. On 8-25-16 Nate and I visited about the history of this situation and the difficulties I have had communicating with Mike with regards to completing the needed items in a timely manner. Nate agreed to be the drinking water contact for the firm and would be my point of contact with regards to the issues needing to be completed. I sent Nate a copy of the 2014 survey and the contact reports to this point. I offered to assist Nate as he worked through completing the needed items. ACTION NEEDED: Finish operations and maintenance manual, create a coliform sampling plan, complete the Emergency Response Plan, and submit plan review.
