| Details: | SUMMARY: follow up to survey deficiencies DETAILS: The system had 6 identified deficiencies notes during the April 2016 survey. Follow up was made with the system and all items have been addressed. An ERP form is on file (contact report done 05/16), ASR on file, all MCL violations have been RTCd. He told me that the O&M manual has been done with further photo documentation to be added later. The well head has been repaired and he will get a photo to me the next time he is at the park. ACTION NEEDED: Deficiencies noted during the survey have been completed. 9/19/16 | ||
