Details: SUMMARY: Level 1 form received DETAILS: A level 1 form was received from the system today 9/23/16. The system has a positive sample on 8/03/16. No repeat samples were collected therefore a Level 1 was triggered. A review of the operations for the sample collection event lead to possible sample collection method contamination. A sample collected 9/7/16 was absent for bacteria. ACTION NEEDED: none at this time
