| Details: | SUMMARY: Survey Deficiency Follow Up DETAILS: Survey on July 7, 2016 noted broken pitless head gasket and loose bolts. System submitted pictures and a statement on October 21, that the gasket had been replaced and the bolts tightened. System also sumbitted a revised coliform sampling plan noting that routine samples be taken from distribution not well. ACTION NEEDED: None. | ||
