| Details: | SUMMARY: deficiency correction date extension - Survey 06/2016 DETAILS: The water system had 5 noted deficiences during the June 2016 survey: Annual survey report not submitted (ASR), backflow device testing not current, no operations and maintenance manual, ERP not complete and no certified op. The system has corrected the two cross connection items and requested an extension for the remaining 3 items. The request was granted with a revised resoluation date to be no later than May 11, 2017. ACTION NEEDED: correct remaining 3 deficiencies by new resolution date - email DWS when items have been corrected. | ||
