Details: SUMMARY: Fu to No Level 1 Investigation Form not Submitted DETAILS: FU to no level 1 being done. Darryl said he was unaware of the positive on the repeat samples. He was emailed an investigation form and he said he will f/u with his lab to see why he wasn’t notified. He was informed he needs to look at his system, fill out form and take more samples this month. ACTION NEEDED: None.
