| Details: | Today, phone contact was made with the system owner/operator. The call today was to discuss the significant deficiencies and rule violations noted during the last Sanitary Survey on May 4, 2016. Per the Sanitary Survey on May 4, 2016, the well does not meet setbacks for the following hazards (OAR-333-061-0050(2)(a)(E):the septic tank is located within 50 feet of the well, household fuel oil tank (now propane)is located within 100 feet of the well, and septic drain fields are located within 100 feet of the well. The above items have not been corrected as they are financially burdensome and impractical for the firm at this time. Per the owner's request, an application for Waiver from Construction Standards was completed and submitted to OHA today, June 2, 2021. Per letter received from OHA dated 6/15/21, the Waiver from Constructions Standards was denied. Contact with the system owner/operator was made on 9/30/21 and 10/06/21, the firm has requested an extension until October 15, 2021 to receive quotes for correction to deficiencies noted on the 2016 Sanitary Survey. Water System Survey Significant Setback Deficiencies letter was emailed to the system operator/owner on October 21, 2021. |
