Details:	Today, phone contact was made with the system manager/operator. The call today was to discuss the significant deficiencies and rule violations noted during the Water System Survey on June 3, 2015. Per the Water System Survey on June 3, 2015, the well does not meet setbacks from the following hazards (OAR-333-061-0050(2)(a)(E): fuel storage tanks, transfer systems, and underground petroleum storage tank are located within 100 feet of the well. The well is not protected from flooding (OAR 333-061-0050(2)): the wellhead casing terminates at grade. Per the manager, the above items have not been corrected as they are financially burdensome and impractical for the firm at this time. Per the manager's request, an application for Waiver from Construction Standards was completed and submitted to OHA today, June 8, 2021.
