| Details: | SUMMARY: Survey Significant Deficiency Correction DETAILS: Called the operator to discuss the correction for the significant deficiency listed on their 2016 Survey for failure to submit the Annual Summary Cross Connection Control Report for 2015. He stated that he had already submitted the report to the state Drinking Water Services. I told him to email me a copy of it and I will forward it to Drinking Water Services.
