| Details: | SUMMARY: Site Visit to Verify Corrections of Significant Deficiency DETAILS: Site visit to verify installation of a new raw sample tap. The lack of a true raw sample tap was noted during 11/19/2015 survey. Met with Butch and a new raw sample tap has been installed in a large valve box between the pitless wellhead and the pumphouse. We also discussed submitting annual CCR and ASR on-time each year. I also gave Butch DRC form to fill out, have owner Dave Hancock sign and return, designating Butch as the DRC. ACTION NEEDED: None. | ||
