| Details: | SUMMARY: Follow Up for Significant Deficiency from July 19, 2016 Survey DETAILS: July 19, 2016 survey identified no gasket for reservoir hatch lid/collar which was located on top of reservoir inside a building, and the top of reservoir covered with dead insects. Met with Butch on site May 4, 2017 and a gasket has been added to preclude contaminants. Further, a pump room has been built around hatch to keep the area clean and most of the insects have been removed. ACTION NEEDED: Continue to keep this area clean and ensure hatch lid and gasket keep out contaminants. | ||
