| Details: | SUMMARY: Contact for Significant Deficiency, Request for 1 Yr. Extension DETAILS: I spoke with Gail Graham and informed her that it is in her best interest to research costs and designs to raise the wellhead and eliminate this deficiency. At this time, I will be requesting a 1 year extension for this work and a deficiency due date for a 1 year period as well. This system is aware that in the event of evidence of contamination, this improvement process will be expedited. ACTION NEEDED: Reset due date to 5/25/2018. | ||
