Details: Background/Problem Identification:
System in need of a plan check package to retroactively “plan check” installed well. RCF also expressed interest in a wavier of plan check fees.

Assistance Provided:
The circuit rider researched the system and the plan check requirements. Contact was made with OHA to determine necessary elements for submission to retroactively “plan check” the new well at RCF. The circuit rider also conferred with OHA with the possibility of a plan check fee waiver. OHA confirmed there is no such exemption for churches.

The following elements are necessary for plan check review:

• Well Log and/or ID number
• A map including the well location and distances to property lines
• Sanitary hazards within 100’ of the well shown on the map
• Photos of the well head/protective structure
• Water system schematic

Attached to this technical assistance contact report you will find the above elements in the same order. Also included is DEQ on-site sewage treatment information from the parcel at 64225 Solari Lane. This is the only possible septage source the circuit rider was able to find that MAY be in the 100 foot radius circle centered on the wellhead location. The information and drawings in the DEQ record is rudimentary at best, and it is recommended that possibly consultation with the septic system installer/homeowner may be the best recourse to determine the exact distance from the septic system infrastructure to the wellhead (likely by pulling a 100’ tape).

Follow-Up Actions Needed:
The circuit rider is available as needed for follow-up assistance.
