| Details: | SUMMARY: Routine Sample Positive for Total Coliform DETAILS: On 10/27 I called and left a voice mail message for the system. This latest coliform is a routine collected after the initial 5 repeat samples all tested positive. The system had been shocked and I explained in the voice mail the system would need to be re-shocked and a level 1 investigation form would need to be completed. I also stated for him to call me back and we would arrange a time for me to come out and help him with the shocking of the system.On 10/30 I received a voice mail from the system. The system indicated he had received a cover letter requiring him to do a level 1 coliform investigation, but not the actual form. Apparently the state did not send the level 1 form along with the cover letter. I called the system back and left a second voice mail, again re-iterating to make an appointment and I will provide a copy of the level 1 form, and any assitance with shocking the system and completing the form. ACTION NEEDED: shock chlorinate system and complete the level 1 coliform investigation form. | ||
