| Details: | Today contact was made with the operator in regards to an unaddressed survey deficiency. A waiver for construction standards form was submitted and denied for this deficiency. At this time, the correction date has been extended as the operator has explained that the facility is in the process of switching to municipal water and sewer in the near future. | ||
