Details:	SUMMARY: Unresolvved WSS deficiencies Notice sent to system DETAILS: Letter mailed on 8/8/16 to the water system to inform them that they remain to have unresolved water system survey deficiencies that need corrected. Mad Dog Country Tavern was to complete corrective action by July 25, 2016 or have an Authority-approved corrective action plan with a reasonable timeframe to complete the corrective actions. To date, this information has not been received for the following three unresolved significant deficiencies:1. Chlorine currently in use is not NSF approved2. No operations and maintenance manual3. Verification of current (not expired) turbidimeter calibration and pH meter standards was not provided during the on-site visit. ACTION NEEDED: Povide public notification to all persons served by the water system within 30 days. Correct water survey deficiencies.
