Details: SUMMARY: Failure to Submit and Report Source Assessment Samples DETAILS: I contacted Mr. Esqueda regarding the failure to submit and report source assessment samples on a monthly basis for a minimum of 12 consecutive months. He will submit a source assessment sample to a laboratory on 1-26-18 and begin submitting the samples on a monthly basis for a minimum of 12 months to include the sample to be submitted on 1-26-18. ACTION NEEDED: Submit and report monthly source assessment samples for a minimum of 12 months to include the current month.
