| Details: | SUMMARY: Water System Survey Significant Deficiency Corrective Actions DETAILS: I received written notification on 12-14-17 that the drum of acetone and other containers with unknown contents ("debris") were removed by 11-22-17 and that the results for the arsenic test were received by the system on 10-2-15. On 3-26-18, I received written notification that raw water sample taps were installed on 12-28-17-- the plan review conducted by DWS for well L114621 showed that the well was inadequately constructed, monthly raw water source assesment sampling is required for 12 monthsbeginning in March 2018 (letter from DWS dated 2-6-18), and the well driller has been notified to make the necessary repairs-- the Emergency Response Plan and Operations and Maintenance Manual were completed on 12-28-17. ACTION NEEDED: Conduct monthly raw water assessment monitoring for 12 months beginning in March 2018. | ||
