| Details: | SUMMARY: Significant Deficiencies Corrective Actions - Corrective Action Plan DETAILS: Documentation received on 3-28-18 that UV lamp was replaced on 3-22-18 and Emergency Response Plan was completed-- plan for new well placed into service in June of 2016 received by OHA DWS on 4-2-18-- documentation received on 3-29-18 that Operations and Maintenance Manual was completed-- documentation of arsenic test with sample collection date of 3-21-18 received on 4-3-18-- request received on 4-12-18 and approved for corrective action extension to 4-30-18 for back flow prevention device significant deficiency-- request received and approved on 4-13-18 for corrective action extension to 10-13-18 for setback from hazards significant deficiency. ACTION NEEDED: Correct outstanding significant deficiencies and send written verification and dates of correction to the regulatory authority by 4-30-18 for backflow prevention device significant deficiency and by 10-13-18 for setback from hazards significant deficiency. | ||
