| Details: | Phone call with Cindy; she did not receive the voice mail left on 6/30/21 from LCEH regarding taking 3 repeats and one triggered well source sample after the positive TC+ result; cell phone service has been spotty since the fire. She did submit one temp. routine TC sample as described on her postcard sent by OHA-DWS on July 7th, and I notified her that the two other temp. routine samples are due by July 30th and also a L1 investigation report is due to the State by July 12th. I emailed her the L1 investigation report form on 7/15/21. | ||
