| Details: | Today an email was sent to the operator explaining that the system's last 3 repeats and triggered well sample all came back coliform positive. It was also explained that the system is responsible for conducting a level 1 investigation of the system, complete the level 1 investigation for that was attached to the email, then email it back to me within 2 weeks of today's date. 8/31/21 Attempted to contact the operator twice by phone and was unable to leave a voicemail. Sent an email to the operator explaining that we still have yet to receive the completed level 1 investigation form from his system and the deadline is approaching. Explained that this completed form must be submitted to me as soon as possible. Also inquired about corrective actions as the last time I spoke with the operator he had explained that they planned on shock chlorinating the system. 8/31/21 Yesterday the operator responded by email stating that the level 1 investigation was completed and mailed into the address listed on the letter that they received and no copy was made. An email was sent to compliance to verify this. The operator explained that corrective actions had been taken and that they are in the process of installing UV light and undergoing plan review for this process. It was explained that there are only a select few models of UV lights that are approved by drinking water services. |
