| Details: | A corrective action plan for the 2020 survey was received 3/8/21. I emailed Doug on 6/3, 7/7 and 7/29 to follow up on the unresolved survey deficiencies. He was having issues getting the storage photos to me, but was able to email them on 8/2/21 to confirm the air vents as screened and access hatches secure and watertight. We discussed the sanitary setback issue with the unused domestic well within 100-feet of the Lafayette Well. I received well logs on 7/30 and 8/18. Doug verified the well log information which was forwarded to the geologist for review. The deficiency due date was extended to 9/15 to allow for the review process. 10/4/21: Email with Doug about setback waiver option for Lafayette Well and request clarification of well logs. On 10/22, information emailed to him to initiate waiver application process. Due date extended to 12/31/2021 to further evaluate and resolve setback issue. |
