| Details: | I spoke with Kathy regarding survey deficiency status and level 1 investigation. There are still 2 items that have not been taken care of: O&M manual and recording of free chlorine residual. She said she has been checking the residual but not writing it down. I encouraged her to get a small notebook to keep with the test kit so she can write it down and keep everything together. An email will also be sent to the owner to see if she can help get these items addressed. A level 1 investigation was due by Aug 21 and I inquired about that. Kathy said they did fill it out and mailed it in but there has been no recorded receipt of it. Another Level 1 form was mailed out today (9/9) to Kathy with a stamped/addressed envelope attached. I also inquired about the lead and copper consumer notification. It was not done in 2018 and not sent out yet for the 2021 results. A letter template/certification form was emailed to both the owner, Shana Reno, and Kathy on 9/10. |
