| Details: | Coli16430 not on SDWIS- Talked to Marry told here to collect 3 RP samples in the distribution and 1 from the Source. The source is a well and the location for the distribution sampling is the assistance kitchen sink and the lower restroom sink. The facility took sample through a filter mounted on the faucet in kitchen and swivel faucet on restroom sink. The facility had media changed between the last sample and this sample in the nitrate filter system. Because facility has conducted level 1 investigation within last 12 months (November 2017), + Coli samples have triggered level 2 investigation. The facility will be in contact with Nick Alviani and Sarah Puls in regards to scheduling this level 2 investigation. Collect 3 RP samples and 1 from the source location. Schedule and Conduct Level 2 Investigation with-in 30 days. Level 2 investigation has increased sampling to monthly. |
