Details: SUMMARY: 1 repeat sample collected 10/06/06 at the service sink sample site tested positive for total coliform. DETAILS: Lee did not mention this sample when we spoke on 10/09/06 about the previously positive routine sample. Due to the persistent nature of this problem, and the MCL violation that has already been incurred for October, I have asked Lee to use special sampling to isolate the problem and verify that it is resolved. He can then return to routine sampling. 10/16- Per Neilson Labs- Lee collected five special samples on 10/10 at the following locations. 1. Janitor sink (fail) 2. Pump intake (fail) 3. Flow meter (pass) 4. Pressure tank (pass) 5. Source (pass). I called and spoke with Lee about these results. Clearly there is a very low level total coliform. Lee is going to do a comprehensive system shock this week and take more special samples. 10/30- Called Lee. They are currently isolating and shocking sections of the system that do not normally see much use. They are taking storage tanks off line one at a time and cleaning the sediment out of them and shocking them. Lee plans to flush the entire system and then wait a few days to collect more special samples (hopefully by end of week). 11/08- Per Neilson- 3 special samples collected on 11/07 (from pump intake, feed room, and the janitors sink). All samples negative for total coliform. Called Lee. He is going to continue special sampling every week to increase confidence that the problem is actually resolved. I asked Lee to submit a routine sample as well so that there is evidence on the record that the problem is resolved. ACTION NEEDED: Lee needs to continue increased monitoring with special samples and also collect a routine sample.
