Details:	SUMMARY: Alert for positive repeat total coliforms DETAILS: Four repeat positive total coliform samples dated 3/17/08. This firm has had several positive results within the past year. Previously the firm has shocked the well and the repeat samples have come back clean but the routine samples during the following month have come back positive. In November a UV light was installed without plan review. The initial routine positive and these four repeats are all with the use of the UV light. The addition of the UV light did not go through plan review. Plan review guidelines were mailed to facility. ACTION NEEDED: Until the system goes through plan review with the drinking water program for the UV light, system repaired accordingly and samples are collected to ensure it is in good working order a cease and desist order is issued for the use of well water used for drinking water, coffee, cappuccino machine, ice, and soda fountain. In addition the well water can not be used to wash dishes. The system will need to notify me when the system has gone through plan review and special samples are absent (this is if you want to go online prior to April 1st). If you are not able to complete plan review and repairs of your well until after April 1st no special samples are needed to restart your system but 5 routine samples that come back absent in April will be required. The cease and desist order is active until then.
