Details: SUMMARY: plant failure DETAILS: Ben called to follow up with the incident last week. They had a power flicker (off / on) at 8 pm on 8/5, and the pump that feeds the plant (called the sight water pump) malfunctioned and shut off. This pumps feed water into the chem feeders (coagulant, fluoride, chlorine). The plant continued to run. Since there was no coagulant being added, the IFE turbidities reached 0.26 NTU and were supposed to go into filter-to-waste mode. Hopefully this is what happened, but they cannot verify that the valve to the clearwell actually closed. Meanwhile, the coag pump was still running, but the feed line had lower pressure since it was empty and there is no backflow prevention device, so it went into that line. This line is also connected to the fluoride feed system, so the polymer 8157 went into the fluoride tank and the undiluted mixture continued to be fed into the clearwell. The DRC was on vacation, so when Ben went to the plant 8/6 at 10 am, he found that the turbidity at the clearwell was 1.782 NTU. The chart recorder was at 0.6 NTU at 5 am, and had flat-lined at 1.0 NTU. By 2 pm the turbidity had dropped to below 0.3. The chlorine residual was also 0.02 ppm at 10 am. They have a strip chart that he hasnt checked yet. ACTION NEEDED: several problems at this plant. Need to figure out why alarms arent working and get back-up systems. Need to measure NTU pre-clearwell. Need backflow prevention devices on chem feed lines. Need accurate as-built plans of plant. DRC operator protocol? Also, needs to do Tier 2 public notice for high turbidity and not meeting CTs. Sent Ben template and HBHs phone number.
