Details: SUMMARY: Failure to submit quarterly sample results DETAILS: 7/29/08 I spoke with Grants Pass Water Lab and verified that a second quarter DI(2-Ethylhexyl) - Phthalate sample was collected on 6/24/08. However the results were not sent prior to 7/10/08 as required. The system operators did not know that they are responsible for forwarding the sample results, not the lab. Chuck Michael at the state DWP returned the system to compliance. I consulted Scott Curry (Drinking Water Program) and verified that because we have received two consecutive quarters of DI(2-Ethylhexyl)- Phthalate samples the system can now go to annual monitoring during the third quarter (quarter of initial and highest detection) of every year. Annual sample will be required for the next three years (2008-2010) per OAR 333-061-0036(3)(G)(I). I contacted the system and notified them of the required sampling schedule for the next three years. ACTION NEEDED: Annual DI(2-Ethylhexyl) - Phthalate samples need to be collected in the third quarters of 2008, 2009 and 2010.
