Details:	SUMMARY: Violation correction DETAILS: August 11, 2009 annual inspection was performed. The operator was interviewed regarding the delinquent coliform sampling. The operator indicated that water testing had been performed on 7/30/09. However, the sample had been submitted under the operators personal name Mike Rice rather than the public water system name and number. The operator was advised to resubmit the results to the state for credit. On September 8, 2009 it was observed that the July 30, 2009 sample test had been registered on the state data base. ACTION NEEDED: Violation corrected.
