Details: SUMMARY: Reviewed on site missing coliform tests and deficiencies as noted on the 03-18-10. DETAILS: System has undergone a change of ownership and missed 2nd quarter 2010 TCR sample. Met with the system on site today to review again all the requirements for monitoring, as well as reviewed survey deficiency items as well. The system now has in place a complete coliform sampling plan, and the start of a operations and maintenance manual. The system still needs to post a Tier III notice for missed samples, as well as provide a screen for the vent on the well head. ACTION NEEDED: Provide a screen for the well head. Take a coliform sample on or before 09-30-10. Complete O and M manual. Post Tier III notice for missed samples.
