Details:	SUMMARY: De-Activating DA DETAILS: OHD DWS had questions regarding absence of test on DA for water system. WSO stated the well DA had high manganese which effected water quality and they discontinued using the well. ACTION NEEDED: Fill out source information form identifying we DA as abandoned.
