Details:	SUMMARY: Received call regarding the Notice of Violation dated Nov. 3, 2010 DETAILS: I received a voice mail message from Harmony at Five Star Bookkeeping on November 15, 2010 regarding the recent NoV letter mailed to David Kinyon, owner of the Mt. Terrace water system. Harmony was verifying that we received a letter mailed by Mr. Kinyon in April of this year which he brought the system into compliance with the rules currently being violated by the water system. I returned Harmonys call on November 16, 2010 and clarified that the water system is not currently in compliance as specified in our recent letter. Mt. Terrace was inspected on August 13, 2010 and continue to not document the chlorine residual as required by OAR 333-061-0036(9). The system has also never submitted source water coliform sampling results as required by a previous letter. Lastly, the system has failed to submit the appropriate paperwork designating Les Gillespie as the DRC operator for the system. ACTION NEEDED: Await response from David Kinyon and immediate submission of required materials from the water system or assess civil penalties.
