Details:	SUMMARY: Follow up from sanitary survey DETAILS: Made a visit today to follow up on items identified during the sanitary survey on 4/21/11. A spigot has been installed and serves as a raw water tap on well head #3 as well as current testing records were provided to show the backflow prevention devices are being tested on an annual basis. The significant deficiency items have been corrected. ACTION NEEDED: none
